Member of nursing team which performs services necessary in caring for the personal needs and comforts of clients under the supervision and direction of a Nurse Supervisor.• Uses the client Plan of Care as instruction for client services.• Communicate effectively with the office, client and client’s family. • Perform tasks which provide optimum personal hygiene for the patient including: • Assists patients with bathing, including shower, tub, or bed bath. • Making up various types of beds. • Combing hair, cleaning nails, giving mouth care, brushing teeth or dentures, giving shampoos, providing skin care. • Encourage clients to follow their prescribed diets; serves between meal nourishments as needed, feeds clients, prepares meals in the client’s home. • Measure and record food and fluid intake and output whenever necessary. • Assist clients in and out of the bed or chair including assisting with mobility using walkers, canes and wheel chair; assisting clients with Hoyer lifts or other mechanical transferred devices as indicated.• Assists in toileting with bedpans, urinals, diapers, Texas catheters and routine catheter care.• Observe and report conditions of concern for patients to the Nurse Supervisor. • Know the importance of and will maintain accurate and complete records of nursing care. Will document services in accordance with agency medical records policies and procedures.• Only share information with appropriate persons maintaining client confidentiality.• Take and record blood pressure, temperature, pulse, respiratory rate, weight and height whenever necessary. • Assist with the self-administration of medicines by reminding or offering pills in pill box.
