Title: Case ManagerSerenity New York is a full-service recovery company based in Manhattan. It offers gender-specific sober living environments throughout the city that provide structure and support while individuals build sustainable lives in recovery, as well as non-residential services including After Care, Intervention, Sober Coaching, and Safe Transport.As a Case Manager, I work directly with the Family & Program Director to develop treatment plans for clients and manage the full cycle of a client’s recovery at Serenity, starting before intake and continuing beyond discharge. My primary responsibilities include:• Identifying goals for each client’s individual recovery• Streamlining communication between all parties involved in each client’s recovery, including therapists, clinicians, and loved ones• Working closely with family members and loved ones to ensure a program of family recovery• Maintaining and implementing clients’ schedules and recovery plans• Monitoring client progress and adjusting plan accordingly• Tracking communication with all parties involved in a client’s recovery• Support and advocate for clients while strictly upholding all confidentiality practices
