Worked with medicaid waiver program on a 45 client case load, helped disabled people find homes, helped get equipment or personal items that medicaid did not cover, coordinated services between us and provider agencies, monitored provider agencies to make sure care was sufficient, set up disabled persons with in home care, typed care plans for each client which where updated monthly, submit care plans to state independently, get psychological treatment for disabled persons if needed, file all paperwork and submit updated plans to providers, visit the homes of disabled persons monthly and quarterly, type monthly reports for visits and monthly phone calls, keep very organized calendar and files for each clients, and independently worked a 45 client case load,
