-Provides intense case management and service referrals for 50 clients, of whom are acute, dually-diagnosed families and single adults. Service provision includes, but are not limited to: rental assistance, parenting skills, mental health & medical advocacy, employment and benefits assistance.-Work with a team in the harm reduction framework to help coordinate services and collaborate with other organizations and service providers on behalf of our clients.-Organized and facilitated engaging life skills groups for clients. -Administer and evaluate client volunteer program. Assist clients with leadership skill-building and increasing their capacity to contribute to the community.-Create monthly building wide newsletter and calendar.-Record client encounters and progress through effective service planning and interventions. Submit reports regarding client objectives and outcomes.-Proficient in Microsoft Excel, Power Point, Outlook and Publisher.

-Provided effective case management for a caseload of up to 60 clients with multiple diagnoses which included referrals, employment opportunities and training, housing assistance, benefits, and medical and mental health treatment.-Facilitated harm reduction based individual counseling and therapeutic groups regarding substance abuse, effective communication, anger management, and life skills for clients recovering from substance use/mental health disorders.-Tracked clients’ progress through effective treatment planning and collaboration with referring agencies.-Supervised and created curriculum primary focused on peer based harm reduction for incarcerated women at SF county Jail -Conducted weekly outreach to distribute hygiene products at Oshun Center, Ark of Refuge and Ella Hill Hutch Community Center.-Worked closely with community members in the Western Addition to eradicate violence by creating mentoring programs for at-risk youth and young adults.-Member of ground breaking community programs such as the Village Project, Maxine Hall Youth Advisory Board, D5 Magic and the Service On Site (S.O.S.) Initiative.-Member of the HIV Prevention Planning Committee with the AIDS Office of San Francisco.

-Created and Coordinated young men’s empowerment group.-Co-coordinated HIV/AIDS Youth Prevention Education Program.-Facilitated comprehensive health education workshops in schools, group homes and after-school programs on STIs; Alcohol, Tobacco and Other Drugs; Body Image; Birth Control; Safer Sex; LGBTQ; Puberty; and HIV.
