I have provided hands-on medical care to tens of thousands of emergency department patients since graduating from residency. Primarily through acute observation, I have developed a deep understanding of the unmet information needs of physicians and patients, particularly in the hospital setting. Clinical information systems can meet these critical needs but only if deployed in an insightful, creative manner. My graduate training has served to confirm my own observations, while giving me tools to address longstanding healthcare problems including patient safety, and translating evidence into practice.My practice has allowed me to explore multiple EHRs, including Epic, Cerner, MediTech, and VistA, and to participate in more than one EHR implementation. This has provided me a close-up view of many of the difficulties encountered in deploying or modifying systems, not the least of which are cultural.

My extensive experience in reviewing emergency department cases for attorneys and insurance companies has given me valuable insight into the quality of health care in the US. It re-emphasizes the need to translate evidence-based practice into all US health care organizations in order to improve patient outcomes while decreasing cost. Informatics, particularly clinical decision support, will be instrumental in this transition to widespread evidence-based medicine.

This phase of my career saw my introduction to electronic medical records as a user, initially MediTech and Cerner, for both CPOE and clinical documentation. It was apparent from my first orientation to an EHR that health information exchange, user experience and patent safety would be challenges for the electronic health records industry. These initial experiences served to motivate me to learn about informatics with the goal of using my clinical background to improve clinical information systems.

As emergency department Vice Chair I was responsible for quality improvement including peer review of ED cases and serving on the hospital-wide quality committee. During my tenure I developed a policy for rapid-sequence intubation in the ED. This included writing the policy and getting it approved, preparing training materials, including a video, and conducting provider (both physician and RN) training. Once implemented, I was responsible for quality assurance according to policy guidelines. My clinical role comprised direct patient care and supervising family practice residents. I served on the Education Committee for the residency program including two years as its chair. The position also required some didactic instruction for residents.
