Key responsibilities• I perform nursing care with high standards and competence to care for the patient while ensuring that organisation resources are used effectively and efficiently in the most cost-effective manner.• I perform daily ward rounds to assess patients, identify their needs and address patient concerns promptly. • I foster effective communication with the multidisciplinary team about all clinical matters concerning the patients’ care. • I provide my patients with essential health care by performing patient/family education on their health condition, treatment, and discharge planning. • I provide nurse-led health coaching and guidance to patients with chronic illnesses to promote confidence in self-management of their health care. • I stepped up to provide care for covid-19 illnesses and coordinate safe and empathetic care for the patients.• I develop an individualised plan of care and documentation for each patient and report all cases that need urgent intervention to the medical team.• I ensure patient safety and quality of care is adhered to through; patient identification, prevention of healthcare-acquired infection and effective medication management through preventing drug errors.• I am a team leader who ensures task assignment is done, carries out supervision of tasks and ensures adequate staffing is done as per skill competence to maintain safe, quality, and efficient nursing service.• I foster cultural competence skills in nursing care to all the patients in the organisation by providing care respectful of their values, beliefs, and culture to promote treatment outcomes and increase the levels of patient satisfaction.• I am currently conducting a continuous quality improvement (CQI) to address delays in STAT medication in my unit -the General medical ward.
