• Assess patient condition during every visit and chart observations• Perform evaluation tasks, including vital signs and medication review and setup• Administer medication as prescribed by the patient’s Physician• Dress or redress open wounds and assess progress of healing• Educate patients and their families on proper home health care strategies and procedures• Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan• Provide palliative care as needed to keep the patient comfortable• Make recommendations for devices or tools that might improve the patient’s quality of life• Listen to the patient and respond to concerns or requests • Ensure compliance with Medicare/Medicaid guidelines with regard to patient care and services• Train and supervise PCA, CNA, RN and LPN as required

• Case Management responsibilities including developing, implementing, and evaluating individualized patient care plans• Consistently act as a social worker, advocate for patient welfare, and serve as a liaison between patients, their families, and healthcare providers• Identify clients’ insurance coverage or other sources of payment for services• Recognize and address client risk factors and/or obstacles to care• Communicate the care preferences of clients, serving as their advocate, and verifying that interventions meet the client’s needs and goals of treatment• Screening clients and/or population for healthcare needs• Provide education to the client/family/caregiver about the case management process and evaluate their understanding• Develop professional working relationship with all the organizational representatives, patients, family, and personal care aide, etc. to ensure the best possible care is achieved• Review and evaluate service plans, goals, addendums and service plan reviews to insure services are meeting patients needs.• Assist with the orientation and training of the Personal Care Aide (PCA).• Completes all skilled nurse visits as assigned and submits paperwork within allotted time frames to ensure continuity of care for supervisor to review• Ensure compliance with Medicare/Medicaid guidelines with regard to patient care and services
