• Perform nutritional assessments of patients ranging in age from new born to 19 years old• Evaluate the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record or medical providers• Counsels patient and or caregiver and formulate or modify diet plan• Communicate with physician and/or facility staff regarding labs, pending procedures, outgoing referrals to other medical providers• Educate patient and or caregiver of medical nutrition therapy guidelines, diagnosis, diet modifications, food selection and preparation methods• Refer patient to child nutrition services to begin special diet at school • Create nutritional handouts and special diets • Document nutrition assessments electronically • Oversee ( Mission, R.G.C, Roma) clinics

Roma/Mission Tx.

• Perform initial nutritional assessment of patients to comply with physician’s requirements to begin physical therapy services• Review patient’s eating patterns, obtain body measurements, and review medical diagnosis from referring physician to determine diet order.• Educate patient and or caregiver of medical nutrition therapy guidelines, diagnosis, diet modifications, food selection and preparation methods

R.G.C. Tx.

• Established dietary program, including creating nutrition assessment initial and follow up forms, nutrition guidelines and handouts• Perform nutritional assessments of patients ranging in age from new born to 19 years old. • Counsels patient and or caregiver and formulate appropriate diet plan• Communicate with physician and/or facility staff regarding labs, pending procedures, outgoing referrals to other medical providers• Educate patient and or caregiver of medical nutrition therapy guidelines, diagnosis, diet modifications, food selection and preparation methods• Refer patient to child nutrition services to begin special diet at school • Create nutritional handouts and special diets • Organize/host health fairs• Filed patients charts & scheduling of follow up appointments • Oversee ( Zapata, Roma, R.G.C) clinics
