* Charge nurse for a 124+ patient clinic with 15 chairs and 14 beds specializing in patients undergoing rehabilitation, the acutely ill ESRD patient, and nocturnal dialysis. *Handpicked by the Director of Operations to ensure CMS recertification, train and retain staff, and cultivate relationships with local short and long-term care facilities. *Took initiative by changing clinic layout and schedule, optimizing staffing and resources, prioritizing safety precautions, promoting patient comfort and care, and reforming infection control practices. This resulted in new clinic culture, better patient and staff engagement, and a successful CMS recertification. *Implemented proactive measures such as daily huddles, weekly in-services, and monthly staff meetings; identified and mentored specific ‘champions’ to help drive change; and conducted routine audits to examine processes, documentation, and clinic culture in order to identify and capitalize on opportunities for improvement.*Coordinated and facilitated all clinic operations acting as the main point of contact for patients, families, caregivers, and healthcare providers as well as the clinical team consisting of 4 doctors, 2 nurse practitioners, 2 dieticians, a social worker, 4 nurses, 12 patient care technicians, a unit clerk, and a biomedical team.
