Lead multi-disciplinary team in meeting psychosocial needs through telephonic support of members enrolled in Medicare Advantage program from diverse geographical areas. Completes thorough Needs Assessment, identified local, state and national resources. Collaborates with interdisciplinary team and community agencies. Provides crisis management and emotional support.• Routinely exceeds performance metric expectations for performance reviews, documentation and member contacts • Selected as subject matter expert in Affordable Care Act project• Chosen as mentor for new team associates in order to improve overall productivity and member satisfaction

Provided psychosocial support to children, youth and families under the Hematology/Oncology department. Primary Social Worker for Blood and Marrow Transplant, Sickle Cell disease and Hemophilia Services. Conducted comprehensive assessment of needs in order to address uniqueness of each patient and family as part of overall treatment plan.• Created process for multi-disciplinary review of family strengths and needs outside of traditional medical model• Developed relationships and worked collaboratively with outside organizations in minimizing barriers to patient and family care • Served as family advocate and hospital liaison on community based out-reach committees including Hospice, Children’s Cancer Center and Sickle Cell Association of Hillsborough County• Along with Pastoral Care Department, co-facilitated department and hospital wide debriefings after critical incidents

Supported terminally ill patients and families during end of life care. Oversaw operation of 60 bed residential program for children who have been victims of physical, sexual and emotional abuse.
