Hospice (Largo, Md)

Strengthen clinical quality and overall efficiency of delivery of hospice services, and ensures compliance with company standards and federal, state, and Medicare guidelines.Use clinical expertise to aide in care coordination.Communicate with all staff in the implementation and documentation of agency policies and procedures that reflects quality and delivery of hospice services.Perform periodic review of patient charts, daily activity reports, and field supervisory visits to ensure branch conformity with agency standards, licensure/regulatory requirements, company policy, and professional community health standards.Serve as a member on various committees providing ongoing education and training. Maintain a professional relationship with vendors to ensure quality service and care.

Case managing a case load of 15-20 terminally ill patients.Assess patient appropriateness for admissions, recertification, and discharges. Assess patient status and notify physicians of clinical changes. Interact with clinical team regarding patient care. Educate patients/families on health care needs, conditions, options, etc. Creation and implementation of appropriate plan of care.Maintained patient charts and confidential files.Extensive knowledge of Medicare hospice benefit.

Supervised personnel in preparing, sterilizing, and caring for operating room equipment and supplies.Maintained record of patient history and prepared reports and recovery charts.Accountable for the delivery of skilled professional care to patients based on national standards of practice in the operating room.Communicate effectively with patients, patients’ families and all members of the patient care team.
