As a Performance Improvement Specialist, I use QAPI methodologies and tools to assist my department as well as OPTN members with performance and process improvement. I’ve facilitated a number of collaborative calls, conference calls, as well as process mapping and coaching sessions to help members identify opportunities for improvement. In this role, my goal is to educate the donation and transplantation community on resources available, foster professional relationships, and strengthen the system to increase transplantation and make it more accessible and equitable.

During my time at Health Brigade, I held two roles: Care Coordination Program Manager as well as Patient Care Coordinator. As a Patient Coordinator I educated patients on resources available to them in the clinic as well as the community including Medicaid services and Public Health services. I also functioned as a Pre-Exposure Prophylaxis (PrEP) Care Navigator assisting patients in enrolling in prescription assistance programs (PAPs) and PrEP care programs. As a Care Coordination Program Manager, I provided oversight to all front-end functional areas: scheduling, patient registration, community health work and medication assistance. In addition to managing the front-end functional areas I provided oversight and managed the Pre- Exposure Prophylaxis (PrEP) program and grant reporting to the Virginia Department of Health. As a key stakeholder in the Hybrid (Free/Medicaid) clinic launch I participated in a number of strategic planning meetings as well as internal workgroups.

During my time at Beacon Health Options, I held the Clinical Care Coordinator position where my responsibilities included completing comprehensive assessments for 100+ clients while also facilitating their individualized care team meetings to ensure patient safety. In this role I collaborated with Community Service Boards (CSBs), Behavioral Health Authorities (BHAs), hospitals, and outpatient clinics to ensure patients had adequate services to meet their needs.

As a Guardianship Specialist at Commonwealth Catholic Charities, I acted as a Public Guardian for twenty-five incapacitated persons with dementia, mental health and/or intellectual disabilities. In this role I conducted comprehensive evaluations of the individual’s capacity and assessed their need for guardianship services as well as maintained their estate and financial affairs by monitoring their income, paying their bills, and making purchases on their behalf.
