I wasone of two consultants at the Emergency Department (ED) of Kerry General Hospital. The department sees 33,000 patients per year and has a catchment population of 160,000.During my time there, I led on the following projects:- Setting up a clinical governance structure for the department- Setting up departmental education programme- Working with HR to improve and stabilise departmental staffing and rotas- Liaising with the regional GP training program to return GP trainees to the department- ED lead for the Stroke Unit- Writing an orientation book for new NCHDs- Reorganised access to fracture clinics- Started a programme of ED patient informationWith my consultant colleague, Dr Martin Boyd, we responded to the government's Special Delivery Unit (SDU) and the Emergency Medicine Programme (EMP) by: - Setting up a clinical operations group for planning programme roll out- Working with the IT department to change the hospital's PIMS (Patient Information Management System) to capture accurate SDU data- Changing the ED's patient registration process- Contributing to fortnightly hospital SDU/acute admission meetingsA new ED opened at Kerry General Hospital in April 2012. I was involved in the project meetings for the building as well as leading on procurement for the new department. I have been able to apply my interests in informatics with the selection of a patient monitoring system which led to becoming involved in the national procurement process for hospital monitors.

UCH is the flagship hospital of University College London Hospitals NHS Foundation Trust. It is 700-bed city centre, tertiary referral, university teaching hospital. The emergency department at UCH treats over 100,000 patients annually and its location of UCH ensures a large caseload from the local residential population of Camden, Islington and Westminster boroughs, mixed with a large volume of transient patients, such as tourists and commuters. In this job I was one of a team of seven consultants in a department delivering consultant-provided care, with a consultant presence from 8 a.m. to 10 p.m. each weekday and rostered weekend hours. There was a full team of clinicians and support staff, including seven emergency and advanced nurse practitioners and four higher specialist trainees in emergency medicine. UCH, as part of University College London Hospitals NHS Foundation Trust, is a teaching hospital for University College London.My responsibilities at UCH included the following:- Management of the emergency department, including staff management and recruiting, training, budgetary issues, audit, and dealing with patient issues and complaints. This required to demonstration of informed decision-making and to ensure that decisions and procedures are properly documented.- The roll-out of the department’s first Clinical Decision Unit.- Departmental lead for Ultrasound.- Attending and contributing to Clinical Governance meetings. - Introduction of structured educational programme for middle grade / registrar doctors- Departmental lead for recruitment of middle grade / registrar doctors, including leading on reviewing hospital practices for recruitment and retention- Rota management for consultant and middle grade staff.- Revising the introductory educational materials for SHOs.- Reorganisation of the department’s review clinics.

My four and a half years as an SpR in London provided me with a huge range of experience not only in emergency medicine, but also in learning how to work in a different healthcare system. In this post I undertook an M.Sc. while fulfilling duties as an anatomy demonstrator. The research done during this year was submitted in my thesis Study of intraoperative blood flow in the portal venous system before and after clamping of the gastroduodenal artery. I was conferred with my MSc degree in June 2001.
