Assesses patients for HIV risk factors using appropriate protocols and assessment tools.Coordinates HIV and STI testing in accordance with agency/program protocols.Coordinates linkage to care with the assigned Prism Health North Texas (PHNTX) medical providers for HIV negative patients assessed to be at high risk for HIV infection.Facilitates all necessary steps for patients prescribed PrEP to obtain medications and to adhere to treatment processes.Provides risk reduction counseling to all patients enrolled in PrEP.Assists all eligible patients with insurance assistance and/or patient assistance programs.Schedules all patient follow-up appointments, including lab appointments.Conducts evidence-based interventions as required.Conducts brief HIV/STI risk screening.Makes appropriate referrals for additional services based on patient acuity score.Creates individualized care plans with appropriate referrals depending on patient acuity score and identified needs.Works closely with clinical staff and PrEP Program Manager to ensure patient compliance with all program criteria and patient graduation from program.Ensures that all patient documentation is entered in the Document Portal and Centricity according to established protocols.Enters data into requisite databases.Submits necessary reports and other documentation to PrEP Program Manager.Other duties as assigned by supervisor or program director.

Conducts HIV and STI testing in accordance with agency/program protocols. Completes patient assessments as related to readiness to engage in medical care. Provides strength-based counseling based on the adapted Antiretroviral Treatment and Access to Services (ARTAS) intervention to ensure patient linkage to medical care. Works with patients to develop and implement short-term care plans to identify and address needs and barriers; provides referrals as needed. Provides risk/harm reduction counseling and education to patients in accordance with agency/program protocols. Assesses needs and makes appropriate referrals for behavioral health, testing, medical treatment, case management, PrEP, and/or risk reduction counseling. Follows up to ensure patients attend medical appointments. Assists patients as needed to enroll in health insurance through the marketplace. Utilizes tangible reinforcements judiciously to decrease barriers to accessing care, attendance at appointments, and follows appropriate protocols regarding inventory control. Maintains flexibility with regard to meeting patients where they are most accessible. Follows protocols regarding timely documentation of each patient encounter and necessary reporting. Links patients who test positive for syphilis, chlamydia, gonorrhea, and/or HCV to treatment and/or follow-up services. Coordinates care with treatment providers and other internal/external support services. Completes accurate documentation for each patient as defined by agency and program protocols. Ensures that all patient demographic information, session notes, testing results, applicable consents, and referrals are entered into the requisite databases in a timely and uniform manner. Prepares necessary reports and monthly program activity summaries and submits them on time. Performs other duties as assigned by supervisor or program director.

Handle a high volume of outbound calls placed by the telephony system to Medicare and/or Medicaid health plan members to offer, explain, and schedule a free in-home health risk evaluation.Receive inbound customer calls to schedule appointments and/or perform other customer service activity. Follow the approved script when discussing health risk evaluations with potential members, ensure members understand and are comfortable with the terms of the evaluation.Present a positive and professional face to clients, health plan members, and workforceLeverage Signify systems to schedule providers to conduct in-home or virtual health risk evaluations; schedule providers presented by the system within the assigned geographic area with appropriate time and distance between.Adjust and reschedule appointments with both members and contracted providers, ifrequested; ensures cancellations and reschedules are recorded accurately and thereforcommunicated to the appropriate parties internally.Adhere to assigned shift times and maintain daily productivity requirements.Return voicemail messages from members, work manual outbound calling list, as required.Seek to support requested overtime to meet unexpected high volume requirements.Report member complaints immediately to Member Engagement Managers.Follow departmental procedures when communicating with members.Respond well to coaching and supervision.Act as a subject matter expert for peer side-by-side coaching.
