I am currently working as a Social Worker in the Care Management and Safeguarding Team in Rochdale. My role being to investigate and follow up alerts where adults are thought to be at risk of abuse - ensuring processes and actions are followed and documented effectively, Ensuring information is made available to multi disciplinary professionals and for multi agency meetings to promote partnership working and to minimise risks to vulnerable groups. I have enhanced my knowledge and experience with adults at risk of abuse. My role consists of undertaking mental capacity assessments and chairing best interests meetings, completing CHC screens and partipating in MDT's, working with individuals of all abilities and needs and their families. Completing assessments of need, support planning, participating in planning meetings with professionals, risk assessing and compiling protection plans whilst ensuring the individual remains at the centre of all decision making and planning, their safety is maximised and their wishes listened to and acted upon.Other social work experience has involved hospital discharge, Intermediate Care and rehabilitation, i worked for 6months as a social worker in a Hospice setting - all of which i have found rewarding and has allowed me to.enhance my knowledge base and contribute to my continuing professional development.
