Case Manager
Current• Provide supportive counseling and crisis intervention for a caseload of 15 to 18 clients and their families• Develop individualized client treatment and rehabilitation plans• Facilitate a supportive environment for clients, including assistance with meeting basic needs, social and family relationships• Monitor and evaluate client progress on a regular basis• Assist clients and their families to develop support networks, linking to appropriate services and supports• Ensure continuity of client care by collaborating with the client, family members and involved professionals.• Advocate for and with clients to ensure the highest quality service provisions.• Facilitate hospital admission when appropriate. • Maintain client files (assessments, service plans, progress notes and letters).• Complete all required non-clinical administrative documents (workload and quality improvement measurements).• Attend weekly team meetings and monthly supervision sessions.
