Diabetes Educator-I have relocated to the San Diego area. I am seeking a position in diabetes and chronic disease management, case management or program development.

 Provided diabetes self management training, telehealth services, and group instruction according to ADA and AADE guidelines for high risk multi-cultural clients and families within in Alameda County. Latino and African American participants achieved a > than 2% decline in A1C during a three year period (2002-2005) and a < 3% hospitalization rate. o Case managed a subset of medically, socially, psychologically complex clients. Utilized protocols to assess physiological and psychosocial status and developed individualized plans. o Educated and supported patients/family members/significant others. o Referred clients to outside resources.o Documented and tracked findings.  Designed and marketed a novel case management referral program for individuals with chronic diseases in Oakland, California.o Promoted the health department’s services and capabilities.  Trained Maternal Child Health Public Health Nurses in diabetes and chronic disease management; provided community workshops on prevention, consultation at employee worksite wellness and staff continuing education units.  Constructed and implemented a diabetes self man gement peer education training program for underserved Afghani, Asian and Latino clients. o Mentored peer educators.o Revised teaching strategies for special needs.  Collaborated with nutrition services to develop obesity standards for case management.  Implemented Continuous Quality Improvement (CQI) program for community diabetes education classes and one-on-one self-management training to improve behavioral outcomes, reduce costs and highlight our success. o Coordinated more than 50 diabetes support groups county-wide and health seminars for seniors. Chosen to represent the health department at a medical training in Cuba (2009).

 Created curricula and taught required lecture classes (100+ students) for Penn State University in Wellness (behavior change, forming a plan for good nutrition, cardiovascular fitness, managing stress…) and Human Sexuality.

 Taught classes in nursing theory and clinical practice to LPN’s, and nursing assistants in the hospital and nursing home. Served as an academic advisor for students and member of the curriculum committee.

 Performed duties as a visiting nurse and charge nurse for Medicare Certified Home Health Agencies in NJ and NY; provided health education classes to parochial schools.
