The nurse practitioner for the CVTS department is primarily responsible for managing post surgical floor patients, seeing surgical consultations, obtaining history and physicals and reviewing diagnostic tests prior to surgery. Have also rotated through the Cardiovascular ICU and managed patients during their immediate postoperative period. This included initiation and management of vasoactive infusions, ventilator management, and invasive procedures. Surgeries included cardiac cases such as coronary artery bypass grafting, valve repair and replacement, ascending and descending aorta repair, and orthotopic heart transplantion. Thoracic surgeries included lung lobectomies and wedge resections, esophagectomies, tracheal reconstruction, diaphragm plication, thymectomies, and lung transplantation. As part of the management of patients I ordered and interpreted diagnostic testing, selected appropriate pharmacologic and non-pharmacologic interventions following national guidelines, provided patient education, initiated discharge planning, and arranged follow up care and home care when appropriate. Along with my other nurse practitioner colleagues, I have provided educational in-services and did frequent informal teaching for the nursing staff.

The Critical Care and Trauma Nurse Internship is a nationally known program for new baccalaureate nurses conducted annually at Parkland Memorial Hospital. It is affiliated with the University of Texas at Arlington, with participants receiving 6 hours of graduate credit for participation in the program. It is also approved by the American Association of Critical-Care Nurses. The class consists of 20 outstanding new nurses who compete for admission in a pool of over 300 applicants. I was responsible for clinical instruction primarily in the Surgical/Neuro/Trauma ICU, but also had interaction at the bedside with nurse interns in the Medical ICU, Cardiopulmonary ICU, and Burn ICU. The nurse interns frequently care for the most complex challenging patients, including those with multisystem trauma, cerebral aneurysms and subarachnoid hemorrhage, brain tumors, traumatic brain injuries, spinal cord injuries, orthopedic injuries, acute myocardial infarction, decompensated heart failure, liver failure, acute kidney injury, ARDS, and complex general surgery patients. The clinical instructor requires a high degree of technical expertise, strong critical thinking skills, and an extensive knowledge of both pathophysiology and pharmacology. It also requires excellent assessment skills and positive, effective interpersonal skills. I have written and presented several lectures to the nurse interns as well as nurses in the Critical Care Residency, Labor and Delievery Residency and Medical-Surgical Fellowship at Parkland. I continued to staff in the Surgical/Neuro/Trauma ICU to maintain clinical expertise.
