CurrentProvide recommendations in a multidisciplinary setting to optimize medication regimens in medical and cardiac intensive care units as well as the emergency departmentAssist with the management of toxicology cases presenting to the emergency department by recommending appropriate interventions, monitoring and disposition planningIndependently monitor, manage and develop individualized plans for patients receiving vancomycin, aminolgycosides, warfarin and parenteral nutritionActively participate in inpatient medical emergencies and trauma alerts through bedside medication preparation and the provision of critical medication recommendations adherent to advanced cardiac life support algorithmsOptimize antibiotic regimens and discuss revised plans with providers and patients in response to updated microbiology labs collected in the emergency department and urgent care settingsParticipate in medication reconciliation on all admitted patients through chart review, inter-facility communication and patient interviewVerify medication orders and visually inspect doses prepared by pharmacy techniciansProduce clinical presentations for training fellow healthcare professionals on updated guidelines, best practices and general pharmacologyInteract with, educate and mentor pharmacy students during advanced pharmacy practice experiences
