Brokered community resources/services and developed resources where gaps existed. Developed a community planning and review (PRT) team consisting of human service organizations, law enforcement, service providers, etc. Coordinate monthly PRT meetings in order to update each offender's case management plan and coordinate referrals/services. Conducted pre-adjudicatory and pre-release assessments and furnishes courts with social and background information concerning a juvenile’s home, school, and community status during judicial proceedings. Provided intake services for juveniles entering the juvenile justice system including receipt and processing of referrals, crisis assessment, and detention screening service. Developed plans for services based on pre-sentence assessments, case history data, court orders and input from other staff members. Made juvenile and family referrals and acquires placement in local or statewide facilities for short-term emergency and/or long-term placement. Conducted pre-sentence assessments of institutionalized juvenile’s home, school and community status to determine suitability for juvenile’s return and for help in formatting aftercare plans.
