Building personal networks around people who are socially isolated. Currently working to connect four older individuals with developmental disabilities with the broader community. Specializing in care for older adults with disabilities who have elderly or recently deceased parents/caregivers.

•Developing navigation action plans to assist patients with aligning their priorities with realistic goals within reasonable timelines.•Assisting patients and their caregivers with identifying and assessing options, which may include accessing necessary services, care, and resources when a patient chooses to live in their own home, rather than in a facility or hospital. •Working as a central point of contact to streamline and record information gathered from family members, and multiple agencies, including hospitals, doctors' offices, health units, and community service providers.•Attending meetings and appointments with patients to ensure that all information, often overwhelming, is accurately documented and relayed. •Advising patients and their caregivers of the equipment and technology available to support the patient’s well being.•Facilitating family teams to provide the best care possible for their loved one when a health event or change of health status occurs.•Helping families recognize their changing needs when a family member is no longer able to participate in family matters.•Finding the fit between a patient's competence and dependency to ensure that they are living their best life. •Supporting decision making and action when the choice to live at home is no longer possible, including the need to sell a home, pack up a home, and moving to a more supportive environment.

Vancouver Coastal Health •Organizing and facilitating focus groups at UBC, VGH, and Richmond General Hospitals. •Gathering and collating information pertaining to the perceptions of practice environment of various employees in the designated hospitals.•Analyzing project results and preparing written reports.Louis Brier Home and Hospital•Conducting a comparative analysis of assisted living facilities in the Lower Mainland which involved constructing report objectives with client, site visits to various facilities, compilation and analysis of data, and preparation and presentation of summary report.•Collating (SPSS) and analysing the data collected from comprehensive resident satisfaction survey and reducing conclusions to written report, including recommendations, for the hospital administration. •Interviewing Resident Care Aide program coordinators at post-secondary educational institutions throughout the Lower Mainland in order to provide resources needed to establish training program credibility criteria for human resources application.Sunset Towers Advocacy and Resources (STAR)Program Developer and Administrator•Implementing and coordinating outreach programs to enhance well being for a disparate tenant mix comprised of seniors, people with physical and mental disabilities, and the “hard to house”.•Implementing and managing a meal program for more than one hundred tenants.•Recruiting, scheduling, and supervising over 75 STAR program volunteers.•Supervising the Assistant STAR Coordinator and S.F.U. practicum student.•Preparing monthly and yearly budget and performance reports for the Vancouver Richmond Health Board, BC Housing, and for the West End Seniors’ Network.•On-site project team member and liaison for the Sunset Towers Revitalization Project, a collaborative, built-environment initiative with BC Housing and the Vancouver Richmond Health Board.

Providing regular, overnight, respite care for adults with developmental disabilities and children with special needs, including palliative care, in our home (up to 60 days/nights/year).

Pilot project, working in collaboration with the Fraser Northwest Division of Family Practice to provide an assessment and navigation service to frail and isolated patients living independently in the community. By identifying and closing gaps in the current public system, the ultimate goal was to keep patients from repeated hospitalization and to decrease visits to the doctor's office.
