 Project Manager of a multi-year, award-winning, collaborative project to bring 13 Community Support Service agencies in Huron Perth together as the Community Support Services Network (CSSN) to act as one integrated sector with shared functions including a centralized intake, shared coordination of care and a shared client record. Led the development of the conceptual model and managed the operational implementation of program components, including staffing, operational space, equipment, budgeting, policies and procedures, evaluation and continual quality improvement. Provided change management support and managed stakeholder relationships between 13 participating partners, including executive and operational leads and operational staff. Worked with legal counsel to negotiate service agreements between participating agencies. Supported and informed the spread of this model across 52 agencies in the SouthWest Local Health Integration Network.  Oversaw the development and continual improvement of a shared electronic health record for 13 Community Support Service Providers using Client Relationship Management (CRM) technology to enable centralized intake, improved communication, shared service data and shared record of care within the sector and to integrate client records across 10 service delivery databases. Negotiated elements of a shared record between multiple agencies, including data set up, interface requirements, data governance, data analysis, and documentation standards. Worked with legal counsel to develop and negotiate Data Sharing and Network Service Agreements.  Project Manager for the development, testing and implementation of a Home Care Collaborative in Huron Perth with 6 Home Care Service Provider partners to redesign how home and community services are delivered in the Huron Perth and Area OHT.

Defined,analyzed and took action on health issues utilizing key health promotion strategies including social marketing, health education and communication, community development and mobilization, program planning and evaluation, policy development, partnership building and advocacy. Focussed on healthy community development, partnership development and healthy land use planning principles.

Instructed post-graduate Therapeutic Recreation students on the nature of disabilities.

Utilized a wide range of interventions and techniques to improve the physical, cognitive, emotional, social and leisure needs of clients. Assisted clients to develop skills, knowledge and behaviours for recreational and community involvement. Assessed patient skills, needs, interests and values related to recreation and leisure, developed intervention plans and programs, delivered programs for individuals or groups, maintained records about client progress and communicated with the health team, and evaluated the success of intervention plans and programs.
