CurrentDirect patient care, performed in Outpatient setting within ILF, ALF, memory care campus and local community, including evaluation, development of appropriate plans of care, ongoing assessment of between 30-40 patients at a time, supervision of assistants and personal trainer, chart review, and communication with patients, family, and interdisciplinary team as well as administrative staff. Subcontractor for Amedisys Home Health services within the Five Star community. Director of Rehabilitation (1/2016-6/2018) Total clinic operations including direct patient care, onboarding, training and supervision of staff, scheduling, case management, billing, private insurance and Medicare reimbursement knowledge, documentation, chart auditing, interdisciplinary team interaction, preparation and updates for DHEC compliance, and customer service.
