Founded new baby food company in San Francisco focused on supporting parents transitioning their infant onto solid food. From purees to baby led weaning, I help parents choose a plan that works for their particular family. I also provide personal chef services for infants and small family events. My primary responsibility was to assess clients in their home, treatment center or long term care facility and determine comprehensive service plan goals to allow clients to reside in the community. Our clients were admitted to the Community Living Fund executed by the Institute on Aging in San Francisco county. Clients typically presented with physical, mental and emotional disabilities often with co-occurring barriers such as substance abuse and homelessness. I maintained a case load of 20 clients whom I managed to secure financial assistance, establishing community health care relationships, securing IHSS workers and hours, job placement, affordable housing as well as overseeing treatment goals and enrollment in out patient or group psychotherapy.

Provide safe and person centered acute psychiatric stabilization for homeless or formerly homeless severely mentally ill individuals w/wo co-occuring substance use dependency. Help individuals build a foundation which includes financial entitlements and connection to out-patient psychiatric and medical service providers for longer term treatment or re-entry into the community through social rehabilitation model. Provide daily treatment groups focused on seeking safety, relapse prevention, healthy communication and management of psychiatric symptoms and coping skills. Performed thorough biopsychosocial assessments and developed measurable and attainable treatment planning with clients.
