Key successes:- Improving >18 week performance and reducing number of open pathways in the service by 30% from December 2015 - April 2016; a result of stringent recording procedures for all admin staff and proactive monitoring - Reducing DM01 (diagnostic) breaches in the Sleep Service to zero by August 2016 by the addition of 'forward-look' practice amongst service administrators- Altering the team structure to include an Outpatient team leader
