Work directly with members as part of a multidisciplinary care team to provide a high quality healthcare experience. Integrated with specialty care clinics across a variety of disease states (oncology, endocrinology, rheumatology, gastroenterology, neurology, infectious disease) to facilitate seamless care. Review new therapies for appropriateness, make therapeutic recommendations, submit for insurance approval and address denials as appropriate, and provide ongoing clinical support.

Review patient prescriptions and profiles to ensure appropriateness of therapy and optimization of outcomes for complex disease states. I participate in all aspects of care coordination including initial intake, insurance processing, medication fulfillment and shipping, patient counseling on therapy and disease states, and clinical assessments. I effectively communicate with healthcare providers to report issues with therapy including efficacy, appropriateness, adherence, side effects, and drug interactions. I am involved in the PGY-1 program, development of clinical initiatives, and reporting of pharmacist value and outcomes. Currently licensed in 4 states, including Oregon, California, Kansas, and Oklahoma, with willingness to obtain more as necessary.

Completed 12 month PGY-1 residency as the first resident at Ardon Health. My learning experiences were focused in specialty pharmacy care, pharmacy administration, managed care (at Moda Health), and ambulatory care (at The Portland Clinic). In this role, I gained proficiency in complex disease states (psoriasis, rheumatoid arthritis, inflammatory bowel disease, multiple sclerosis, hepatitis, HIV, oncology, mental health, etc) necessary to work autonomously. I provided telephonic patient counseling and education, completed technician data and filling verification, developed and updated patient management programs, and collaborated with leadership to assist in implementation of a new pharmacy software. As the first resident I helped to develop the program and apply for ASHP accreditation in Spring of 2018.
