Identification of needed interventions was carried out through leveraging the expertise of Physicians, Licensed Counselors, Dieticians, Nurses and Exercise Physiologists serving target population. Submissions of results were reported to the governing agency in required increments. This three-year endeavor created multiple successes for many of the participants.
