I'm currently leading the LHS Collaborative a stakeholder-driven follow-on effort to execute this Roadmap through learning communities focused improving care and outcomes for clinical conditions such as hypertension, chronic kidney disease, pain/opioid use, and sickle cell disease (https://drive.google.com/file/d/13dG3nt7Yge7wjvynR9Dn9UyeS_wHecIq/view?usp=share_link).Guidance I've developed to support care transformation at the care delivery organization level includes creating the CMS-recommended "Clinical Decision Support Five Rights framework," (bit.ly/cds5rights), the HIMSS guidebook series for improving outcomes with CDS, and the online "Guide to Improving Care Processes and Outcomes" (bit.ly/qiguide) provided by the federal agency that supports nearly 1400 health centers in providing care for 25 million people. I've helped dozens of care delivery organizations apply quality improvement (QI) strategies and tools to successfully transform how they deliver care for selected targets and beyond.
