Support Coordination & Supports research - assist participants to build the skills they need to understand and use their plan- work with participants to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in the community.

Coordinate the lifestyle, therapy and spirituality programs at the home ensuring our residents live their best life through providing choice, encouraging independence, enhancing the experience and wellbeing of residents. LT&S Coordinator is required to coordinate plans, monitor, evaluate and lead a team of staff and volunteers to implement person-centred programs which promote the physical, emotional and spiritual wellbeing of residents through the involvement in meaningful activities.

Perth and Goldfields/Wheatbelt representative responsible for promotion, education and advice with regards to Regional Delivery Partner (RDP) services for the federal Integrated Carer Support Services (ICSS) initiative known as the Carer Gateway which incorporates providing supports to carers identified in the Carer Recognition Act who provide daily care and support to people with disabilitiy, medical conditions, mental illness or who are frail aged.

This role is responsible for referral, intake and caseload management of NDIS clients and works as a critical interface between the organisation and the customer. The CEC ensures an exceptional customer experience is delivered throughout the customer journey, connecting them with the relevant services and passionate teams along the way.
