Education for staff, primary caregivers, and community leadership regarding the volunteer benefit within Hospice care. Ensures timely delivery of services by team.

Build volunteer program from the ground up to meet and exceed hospice Medicare requirement for 5% of direct care hours provided through volunteer services.Interviews, onboarding, schedule management, and ongoing training for over 50 volunteers.Responsible for complying with and training volunteers on infection control regulations throughout the COVID-19 pandemic.Manage Volunteer Services by tracking referrals and providing documentation for a census of 250+ patients at one time. Working closely with multiple disciplines to match volunteers with patients based on individual and family needs

Process referrals and orders for Primary Care, Home Care, and Hospice patients in a full continuum of care setting.Follow patient transitions to/from the Hospital and TCU by calling for Nursing/Social work updates and requesting records at time of return.Assist with schedule maintenance and office work for the Providers and Clinicians that work with Optage Primary Care, Home Care, and Hospice.Medical records/Patient chart maintenance and release of information.Provide effective communication between Patients/Residents and their Providers/Clinicians.

Manage the reception desk and be the first point of contact for residents and family members to the staff.Answer all phone calls, provide answers to needs and questions, and direct specific needs to other staff members.Anticipate and handle all office issues that arise and attempt to keep things running smoothly for other staff.Scheduling appointments/meetings and general staff calendar maintenance.
