Personal Home Care Assistant for several clients ranging in ages from 6 to 85.two that clients that are on vents and have other challenges with communication. It is an extremely rewarding job and the clients and their families couldn't provide a more rewarding career for me.

I have done photography for over 15 years with several companies and on my own. I previously had some of my work on display at Great Rivers Art Gallery in Hastings Minnesota until they closed. I do all venues of photography including,portraits,senior pictures, landscapes and weddings to name a couple.

I work hand in hand with the nursing home (2 facilities) staff and Allina Physicians and NP's to ensure the best care. I provide updated clinical information on each patient in the TCU and make sure that the EMR reflects all the new information regarding there stay in the TCU as well as update medications, vitals, allergies, labs, and other procedures. I plan and schedule the Providers schedule's for two different facilities. I make sure the Nursing home staff and patients have the appropriate notification of the providers visit. I attend bi-weekly meetings with all departments to obtain the overall condition of the patients and the discharge plans for them. I am involved in pilot projects that I helped to initiate and now are implementing to track home care services being offered to TCU patients , the amount of acceptance of that service, and the overall effective on readmission.I assist in making sure all appropriate departments are notified of a patients discharge so the patient will get everything they need upon discharge and any home services are in place.I am available to the patients , patient families and staff for questions. I act as the liason between the provider and patients, Nursing staff and patient families as well as other employees.Providing the best care and support to the everyone involved and to assist in maintaining the quality of life is my daily goal.
