Rotation summary/highlights/accomplishments: - participate in daily rounds with unit teams and make suggestions to optimize pharmacotherapy; perform BPMH/MedRec for 5-10 patients weekly - monitor lytes/organ function and adjust TPN for adults/neonates accordingly - participate in antimicrobial stewardship by narrowing antibiotic therapy when able - monitor vancomycin/gentamicin TDM levels for all patients on ICU, pediatrics, NICU, and maternal/newborn and adjust vancomycin dose/order trough levels accordingly - create care plans for patients with a variety of medical conditions (e.g. opioid overdose, blastomycosis, acute respiratory distress syndrome, etc.) - attend interprofessional teaching sessions with various professions (medicine, nursing, dieticians, etc.)

Rotation summary/highlights/accomplishments:- patient population: methadone/Suboxone, students, travel medicine, HIV+ individuals - provide vaccinations (Engerix, Shingrix, Gardasil9, etc.) to patients - complete MedsCheck reviews for a variety of complex patient cases - provide training on various diabetes glucometers - respond to multiple OTC inquiries every day - product check, clinically verify, and/or counsel hundreds of medications daily (250-300 prescriptions daily) - compound medications as required

Rotation summary/highlights/accomplishments:- primary client population included individuals with mood disorders (personality disorders, bipolar disorder), depression, and/or anxiety - pharmacist/student were consulted by a variety of professions on the unit including psychiatry, social work, nursing, occupational therapy, etc. to provide medication recommendations and/or education - role at site was to assess clients enrolled in the Integrated Day Treatment program who could benefit from medication optimization (CBT/DBT program which clients attend multiple days per week over multiple weeks) - main responsibilities: attend interprofessional client rounds twice weekly, work up clients’ medication histories, provide medication recommendations to clients and/or other healthcare providers (e.g. psychiatrist), create evidence-based, client-friendly information on medications, respond to drug information questions from clients or healthcare providers, attend weekly educational grand rounds on topics like alcohol withdrawal, etc.

My role as a pharmacy student at this site included the following activities:- technical aspects of pharmacy, including data entry, filling, and dispensing medications - compounding topical and oral medications - dispensing methadone and Suboxone - performing MedsChecks - interpreting INR measurements - creating and checking blister packs - collaborating with various health care professionals including family physicians, physiotherapists, and specialists (e.g. paediatrician, dermatology clinic) in delivering optimal, patient and family centred care- maintaining patient confidentiality and upholding all NAPRA Standards of Practice

As a pharmacy student I was responsible for the following roles: - updating the hospital's parenteral manual protocols - working with floor pharmacists to perform Best Possible Medication Histories and Medication Reconciliations- performing a hospital wide audit of all BPMH/MedRec data in patient charts- collecting and compiling chart data - creating visual graphics to compare pharmacist, physician, and nurse performance with respect to level of completion, timeliness, and accuracy of BPMHs/MedRecs - presenting this data to various members of the TBRHSC - destroying narcotics- completing narcotic count
