Psychiatric nurse - LAC DHS Office of Diversion and Reentry, project-based permanent supportive housing: While at The People Concern, I served as a nurse case manager working within a team of professionals providing intensive case management services to our clients in permanent supportive housing. I provided psycho-social education regarding diagnoses, medication treatment and self-care and supported the psychiatric team with client status and clinical observations. Other routine nursing duties included ordering, reconciling and administering medications and injection and maintaining patient records via the ODR Database and CHAMP. In addition, the psychiatric and primary care team relied upon my administrative strengths in coordinating and scheduling psychiatric appointments. This role involved ongoing medical case management: facilitating primary care appointments, lab orders and tracking hospitalization discharge and treatment plans for in-home care.
