As a Key Worker Specialist, it is our role to ensure that best practice is delivered to our families. This requires a family-centred approach that focuses on tailoring support to the individual needs of the child and identifies the type and level of early intervention supports that a child and their family needs to facilitate the best outcomes.Responsibilities:- Assessing children aged 0–6 years in their natural environments, providing recommendations for early intervention supports and services appropriate for the child’s development and social participation.- Assist and support families by building their capacity to meet the child’s developmental needs in their natural environments - home, ECEC/school or community. - Connect with and provide support to mainstream and specialist services to assist them with inclusive practices for children with additional needs- Maintain accurate records, attend planning meetings and prepare reports in accordance with NDIS stipulations- Manage all NDIS engagement including goal setting, budget, professional services, accounts, ongoing reviews and case collaboration with supporting services.- Liaise and engage with referral agents and community organisations to build professional relationships aimed to promote positive and enriched outcomes for our clients.- Coordinate information flow to and from families, team members, referral agents and community organisations- Attend interagency meetings/ forums and educate community Stakeholders about our service.- Build rapport with child and family, and act as the advocate for participant, family/carers.- Represent the participant with all school interactions and support ongoing development within the community.- Facilitate referral pathways for families with other service providers

Working with families and educational facilities to support the inclusion of children with disabilities in their educational setting and ensure adequate provisions are being made. This work utilises my skills as an experienced educator.Keyworker intake meetings with parents resulting in liaison and service engagement with allied health professionals, medical professionals, educators, Community Health and Human Services to ensure adequate service provision for children with disability.Provide training in Keyword signing to parents and educators to promote inclusion.Teaching Social Skills groups to children experiencing pragmatic difficulties.Facilitating School Readiness groups for children with delay development or disabilityDevelop and deliver parent education in school readiness and social skills development.Responsibilities: - Develop the eight-week school term programs - Facilitate groups after school weekly- Provide strategies for parents/carers and aid skill development - Provide training evenings for parents/carers- Liaise with educators, conduct school visits and meetings- Liaise with allied health, education and medical professionals
