Farm to Patient is a fresh produce prescription program working with patients in the community whose doctors have advised them to change their diets as part of a comprehensive treatment plan. Fresh Rx is committed to buying produce from local family farms that use organic growing practices; that means all the produce is grown without the use of synthetic pesticides or fertilizers. Participants receive a free 16- week farm share which allows them to start incorporating more fresh produce into their diet without adding any additional financial pressure. The farm shares typically include microgreens and a seasonal selection of locally grown vegetables.Assisting with collecting, managing, analyzing and reporting on Farm to Patient Program Data since 2019 with over 400 surveys collected.

This includes updating, designing and administering surveys to various key stakeholders involved at El Sol, as well as creating reports for a variety of audiences. Conducted El Sol's 2016 Annual Client Survey. I updated yearly survey format, recruited and trained 15 volunteers on appropriate survey/interview methods. Conducted the survey for 1 month, and received 127 responses. Then I entered data into local databases for easy access for co-workers, and wrote a comprehensive report to be distributed to staff members, board members, volunteers and community partners.Created and updated data collection methods for El Sol’s strategic planning process. I have created, updated and revised 5 surveys to be distributed to various key stakeholders (clients, volunteers, Board of Directors, staff and community partners) to better understand our relationship with key stakeholders, our role in the community, and identify areas of improvement.
