Living Independence for the Elderly (LIFE) is known as PACE (Program of All-inclusive Care for the Elderly) nationally. LIFE is a community-based program providing dually-eligible, frail enrollees over the age of 55 with all necessary health services, including primary care, restorative therapy, and other services while they stay in their homes.As a Nurse Practitioner I provide primary care to LIFE participants at on-site clinics in Chambersburg, PA and Enola, PA. - Types of visits include: semi-annual assessments, post hospital and skilled rehab discharge visits, medication management and acute care visits. I perform on-site procedures such as joint injections, laceration repair, I & D, skin biopsies, gynecologic, ENT and podiatry procedures.- I collaborate with the clinical pharmacy team in monthly pharmacy reviews and Medication Risk Mitigation.- Serve as a member of the Interdisciplinary Team (IDT), working to design an individualized plan of care for each participant.

Wellspan Pulmonary & Critical Care at Gettysburg Hospital ICU. Conducted Tele-Rounds via video-link with a remote Intensivist while providing on-site support to hospitalists and specialty providers.- Management of acute and pre-existing conditions. - Order pharmacologic, diagnostic and non-pharmacologic treatments. - Coordinate with Hospitalists, Specialist and Ancillary care services. - Resource for coaching/teaching support to critical care nursing staff and other team members.- Nexus of communication for patients, families and multi-disciplinary team.

- Initiated full-time outpatient clinic at Gettysburg Hospital. - Built patient panel to over 250 patients. - Managed complex heart failure, post cardiac intervention and arrhythmia device patients in collaboration with cardiology team. - Frequent outpatient clinic visits and telephone management to review/order and titrate medications, lab work and diagnostics. - Conducted IRB approved study and program teaching patients mindfulness and stress reduction as coping mechanisms for chronic illness.

- Provided comprehensive care to adult patients with acute and non-acute medical problems.- Conducted thorough history and physical exam and developed differential diagnoses. - Ordered appropriate diagnostic and pharmacological care and referred appropriately for admission or specialized care. - Minor procedures including splinting, joint and oral nerve block injections, I & D and laceration repair.
