Case Management Supervisor

• Direct and execute Case Management program at local non-profit. • Supervise, mentor and develop contract staff and interns within the case management program ensuring that they are performing according to program needs and best practices. • Provide direct services to clients in need of support outside of therapy or advocacy services, developing goals with clients for safety and stabilization allowing them to function independently. • Utilizing a holistic approach, accommodate three different types of services including: drop-in, intermediate, and long-term case management. • Delivery of support services including HUD funded Coordinated Entry Screening for housing, basic needs, mental/medical health needs, and other referrals to community resources for individuals. • Case management services are client-centered, trauma-informed and are provided for the purpose of facilitating access to, coordinating, and ensuring holistic care. Developing partnerships in the community has been essential to the success of the program.Achievements at the Domestic Abuse Project:• Testified at the House and the Senate to successfully pass a bill that resulted in two years of funding to support a Veteran program at DAP (Domestic Abuse Project)• Designed and developed Safety Plan that has been implemented statewide, providing professional tools for staff to use with victims of violence. • Per HIPAA regulations, led the migration of electronic health records system data base import/export.• Developed partnership with Hennepin County Vets court, increasing referrals to DAP. • Co-Authored Blog: Intimate Partner Violence: A Community Response Model Blog• Coordinated and facilitated various Fundraising events, raising more than $200K annually. • Instrumental in providing support for the 24-hour hot line ensuring a quick turn-around in call back response.
