In this position, I coordinated navigation services for elderly and socioeconomically challenged patients of the health system. While working here, I supervised the SHIP Medicare consultation program, the Senior Fit program, and developed a Community Health Worker program that I found funding to support and integrated this service into the ACO care coordinator team, as well as other aspects of the organization.

In this role, I was responsible for marketing the home care services of Home Instead Senior Care, care planning with families, performing new patient intake evaluations, providing community education and training, raising awareness of senior issues, and advocating for this at-risk demographic. As the Community Service Representative, I successfully sought out and formed new partnerships in the community, with an eye to innovative and creative ways to meet current challenges in the senior healthcare industry. Building such partnerships often required in-depth logistical planning and coordination, while garnering the trust of various collaborating parties.

As the Project Coordinator, I was responsible for representing a federally funded program to the public, performing all administrative and operational functions necessary for daily working of the program, recruiting and interviewing volunteers, preparing, submitting and managing all grants, managing fundraising efforts and overseeing the budget associated with the program. While in this position, I secured a renewal of a three-year competitive federally funded grant.

This position required me to investigate initial reports of child abuse/neglect, involving interviews of children, parents, alleged perpetrators, and other contacts. I kept thorough and accurate documentation, successfully engaged and case planned with families to connect them with appropriate services, communicated with service providers regarding the families’ needs, and presenting findings and recommendations in court.
