Majority of contact and intervention provided in the community with frail elders and their families in context of multidisciplinary assessment and intervention. Targetted care to maintain independence in community by maximizing skills and supports. Functional assessment provided to enable multidisciplinary team to focus intervention on maintenance of community living. Developed and delivered group-based interventions to reduce functional impairment related to major mood disorders, social isolation and mild cognitive impairment. Whole person assessment of independent living also included caregiver support and education related to dementia, home safety, mobility evaluation and intervention.
