Industrial Incident Investigation- Conduct root cause failure analysis of large scale incidents, fire and explosions in the refining and petrochemical process units at the Sunoco Philadelphia. Perform additional incident investigations associated with mechanical breakdowns, unexpected failures, operational upsets, loss of primary containment, first aid and lost time recordable injuries, design flaws in process systems and procedure deficiencies. Develop solutions to prevent recurrence of incidents in accordance with the applicable OSHA PSM standards and all other state and federal regulations.Training- Perform lead investigator training of approximately 90 personnel through initial 32 hour training and 16 hour annual refresher training. Training includes: Root Cause Failure Analysis - Digging into the root of problems and breakdowns which allowed the failure to occur. Communication, Operational , Mechanical or Personal Decision MakingLogic Tree Analysis- Driving the root causes into a logic tree to show the flaws where if removed some factors from the tree could have prevented the event from happening. Tool used to uncover many different safety deficiencies. Near Miss investigations and Following up with the management and hourly staff of the refinery to assure that we are taking steps and putting actions into place to prevent from even a near miss occurring. Training refinery population (800 people) annually- Conducting classes to provide the refinery population with the TOP methodology and share past investigation information,Conducted PSM training to give people the basic knowledge of the 14 elements of PSM Initiated TOP awareness training for the entire facility (1,000 people) – Conducted weekly classes to educate refinery management and hourly employees on the principles of the TOP methodology. Help Comply with OSHA and DOT Regulations Supervision – Schedule and supervise TOP investigator and supervise all of the training
