o Work closely with medical and nursing staff to ensure that patients receive the best treatment, advising on the selection, dose and administration route by conducting medication profile reviews o Participating in ward rounds, taking patient drug histories and involvement in decision-making on appropriate treatmentso Medication reconciliation o Verifying order entry using Meditech pharmacy moduleo Dispensing medication and complying with aseptic techniques o Coordinating the pharmacy technician ´s duties – who work in the preparation, labeling and delivery of various medications

o Provide clinical services to long term care facilities and Retirement communities o Review medication use for residents. Ensure that only the necessary and minimum number of medications is used for each resident. Ensure there are minimal drug-disease and drug-drug, drug/food interactions. Ensure all doses of medications are within desired therapeutic guidelines.o Provide education for the nursing and medical staff as well as the residents and their families.o Participate in the medical advisory meetings, contribute to the accreditation process, and work in an interdisciplinary committee o Perform Medication reconciliation for new admission or hospital readmission o Ensure proper drug destruction for unused narcotic medicationo Ensure pharmacy services are compliant with Ministry standards and work with clients to ensure they are meeting Ministry standards from a medication management perspective
