The Senior Zero Harm Investigator conducts KiwiRail Operation Investigations (Rail and Marine) and works closely with Regulatory Authorities (TAIC, NZTA, MNZ, WorkSafe, and Coroner). The role was accountable for implementing investigation policy and procedures, conducting fatal risk accident investigations and providing expert support and mentoring to Line Managers for safety occurrence investigations. Duties included lead/support investigation, manage accident site, manage SIMEXs, collect and manage evidence, collect statements, co-ordinate and manage queries including OIAs, arrange testing, research, review and analyse evidence, prepare reports, correct action management and conduct RIC training.

The Zero Harm Senior Assessor is responsible for providing specialist support to the Zero Harm Manager Verification and Line Managers. The roles function is to work with the business to develop a robust annual internal and external audit schedule and oversight the business as it delivers the internal audit programme in accordance with the requirements of the Safety Case and National Rail Systems Standards. The roles purpose is to provide assurance to the Executive, through the Zero Harm Manager Verification, in regard to the performance of the KiwiRail Safety Management System.

Safety Investigation Engineers are responsible to the Manager Quality and Manager Safety Investigations for the day-to-day administration of the Safety Management System (SMS) by carrying out investigative functions, risk management, occurrence reporting and notification, regulatory compliance, operator liaison, provision of technical/strategic advice to operational divisions to ensure safe, compliant and efficient operations. Safety Investigation Engineers plan, prepare and conduct investigations and investigation reporting in accordance with company policy. During an investigation the factual sequence of events are determined and analytical risk assessments carried out (environment, organisational, human factors, safety data) to identify trend(s) in order to prevent recurrence (corrective actions). Upon investigation report acceptance the report is then distributed to the business and customer. During the final stage of the process it is the Safety Investigation Engineers responsibility to manage and support the operational division(s) by monitoring and facilitating the closing out of the associated corrective actions.

Responsible for leading a team of engineers and supervisors in the successful completion of maintenance projects as well as price negotiation through to procurement and project sign off, liaising throughout with subcontractors, project team, client and process operators.Generated and advised on company policy, forms and procedures. As an RNZAF ACFTTECH and trade training Instructor I have in-depth training and extensive experience on the following RNZAF platforms C130’s, P3K’s, B727, Andover’s, UH-1H’s (IRO Ground Runner), Seasprites (On-Line Maint) and including the relevant Structures, Systems, Components and Engines (Intermediate and Depot Level Maint). My final role within the RNZAF, prior to retirement, was as a Contract Relationship Manager (Dap Aeronautical Procurement – CRM Aero 1). I was responsible to Aeronautical Contract Relationship Manager for providing procurement services to the RNZAF for logistic contracts and advice on an agency basis for non-logistic activities. Procurement services included all aspects of contracting and strategic vendor relationship management IAW Defence Force Order (DFO) 52 Policy (procurement, purchase, tendering, custody, distribution, use and disposal).
