We also monitored A&E admissions and carried out rapid assessments to determine whether patients were at their functional baseline and safe to return home as the medics deemed them medically fit. The job involved liaison with medical staff, occupational therapy, frailty team and the discharge hub. Understanding discharge pathways and contacting community services such as ICASS, downstream beds and social services was also an important aspect of the role. -

Education included discussions around COPD, breathing techniques, ACBT, clearance techniques and anxiety. -

Onward referral to community physiotherapy and exercise classes on discharge allowed continuity of care. -
