We also monitored A&E admissions and carried out rapid assessments to determine whether patients were at their functional baseline and safe to return home as the medics deemed them medically fit. The job involved liaison with medical staff, occupational therapy, frailty team and the discharge hub. Understanding discharge pathways and contacting community services such as ICASS, downstream beds and social services was also an important aspect of the role. -

Exercise classes were held 3 times per week and my role involved organising and leading the class, as well as providing 30 minutes of education to patients before they exercised. Education included discussions around COPD, breathing techniques, ACBT, clearance techniques and anxiety.

My role involved a thorough subjective and social history followed by functional head-toe, balance and frailty assessments. Patients were assessed with a multi-disciplinary approach from the team involving co-ordination with the medical staff (doctors and nurses), occupational therapy and physiotherapy. Provided education and advice regarding the importance of health, wellbeing and physical activity while increasing awareness around the current physical activity guidelines in the UK. Patients returned for review assessments and guided rehabilitation / strength and conditioning sessions. Onward referral to community physiotherapy and exercise classes on discharge allowed continuity of care.
