Work with a variety of patients within Westview Health Centre providing social work assessment and intervention, supporting patients and families as they navigate supports within the community. Conduct financial assessments, support in family conferences, and provide connections and referrals as needed.

Provided casework, connections, and SW assessment and interventions for Seniors in Parkland County, Spruce Grove, and Stony Plain. Supported in connecting individuals to resources, building capacity in the community, and connecting with service providers to strengthen relationships among partners.

Manage a case load of about 20 individuals to ensure they get quality care and client-centered support. Work with an interdisciplinary team to provide support to clients with intellectual and mental health concerns.

Worked with an individual who had disabilities supporting her in her access to the community and daily living activities.
