This includes connecting individuals with further resources, supporting them with paperwork,… Show more The Salvation Army: Addictions & Rehabilitations Centre, operations surround providing services to individuals struggling with addiction, housing, and other modes which bring about homeless, while serving as the only dry facility in the Greater Victoria Area. My role as a Case Worker surrounds bringing individuals into our facility and supporting them to work on what issues brought them to our facility. This includes connecting individuals with further resources, supporting them with paperwork, and helping them to identify the best methods to address what they are currently struggling with through individualized plans. Through the job I work with three different programs throughout our facility. The 6-month addictions recovery program, in which I am the main support for the program facilitator, helping individuals struggling with addiction learn new skills and develop new habits to better tackle that struggle. The Transitional Housing program to help individuals find appropriate housing, and deal with any other difficulties in they're lives so they can move forward with productive skills, and a proper network of resources. Finally, the emergency shelter program in which immediate shelter is offered to individuals dealing with homelessness. The role also surrounds supporting staff who provide support to the individuals in our care on a daily basis to ensure that they are in a position to best address situations as they arise, and to limit the amount of situations they have to deal with. All of this is done in a collaborative team focused manner, in which the case management team works together to ensure that the facilitate operates a smooth as possible so individuals in our care can meet their goals without unnecessary obstacles. This includes addressing emergencies as they arise, supporting clients through day-to-day struggles, and communicating with other staff to inform them of what is occurring, so they can best address the situation
