- Coach, encourage and support over 35 young adults for greater social inclusion.- Responsible for being aware of youth whereabouts while maintaining a safe and productive environment at all times.- Facilitate problem solving with young adults.- Assist in providing structured group activities/functions when necessary.- Provide transportation to and from various program activities.- Provide and maintains quality documentation, record, reports and evaluations per agency standards.- Comply with agency, federal and state policies, procedures and regulations.- Attend regular training and development opportunities to maintain an up-to-date knowledge of safeguarding, health and safety and local policy developments.- Actively participate on one or more of the following teams: Medical, Supply, Filing, and Cleaning.

- Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting members health and treatment plan through various communication channels.- Build and maintain positive working relationships with the Healthcare members, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Work to reduce cultural and socio-economic barriers between clients and institutions.- Assist with care plan implementation and Healthcare member education during in-home visits, help develop care management strategies, and work with team members to provide linkages for the various health and social needs of members.- Assist members to identify socio-economic issues that affect their overall health and develop health/social management plans and goals.- Identify gaps and opportunities to strengthen systems of care within the members community and assists members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors), and assisting with completion of applications for eligible programs.- Assist with patient medication adherence by: instructing the member on current medication list, reviewing medications with member and assist in obtaining refills.- Teach disease self-management (i.e. nutrition, symptom tracking and reporting).- Accompany members to appointments as needed.- Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner (e.g. progress notes, letters), and other administrative responsibilities as needed.

- Maintain confidentiality of records relating to clients' treatment - Encourage clients to express their feelings and discuss what is happening in their lives, helping them to develop insight into themselves or their relationships.- Develop and implement strategies to meet the needs of students with a variety of handicapping conditions- Maintain accurate and complete student records, and prepare reports on children and activities, as required by laws, district policies, and administrative regulations.

- Employ special educational strategies or techniques during instruction to improve behaviors.- Provide a safe environment for clients preventing self-harming behaviors and or aggressive behaviors to others by utilizing TCI techniques. - Creating a safe therapeutic environment with clients throughout the day. - Communicate with clients to provide them with comfort, encouragement, and positive reinforcement.- Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, or physical abuse. - Teach socially acceptable behavior, employing techniques such as behavior modification or positive reinforcement.
