Supervise, coordinate, manage and maintain Home Health Aide coverage for all clients on a daily basis, in conjunction with the designated contract personnel and/or the Registered Nurse.Responsible for planning, coordinating, managing, and evaluating cases to ensure delivery of greater home care services to patients. Oversee scheduling and assigning patients to nursing staff/home aids to achieve maximum productivity and cost effectiveness standards. Knowledgeable of standards of practice, positions, procedures and regulations appropriate to home care. Coordinate/Manage home care visits, when necessary, and submits corresponding documentation regarding patient and each visit.

• Responsible for data entry of all patient services into 3M system• Verification of accuracy and assigning patients to clinicians within CDS• Processes 485 (MD orders) and notes from clinician• Responsible for arranging aide services, rehabilitative, and nutrition services and equipment delivery for patients • Communicates with patients to obtain any precertification/verification or referrals required at time of service.• Effectively communicates with doctors and other team members any patient care limitations based on precertification/referrals.• Acts as a liaison between HHCP and patients
