• Develop rehabilitation plans that fit clients' aptitudes, education levels, physical abilities, and career goals. • Maintain close contact with clients during job training and placements to resolve problems and evaluate placement adequacy. • Locate barriers to client employment, such as inaccessible work sites, inflexible schedules, or transportation problems, and work with clients to develop strategies for overcoming these barriers. • Develop relationships with employers in the community and explore employment opportunities for client base

• Manage caseload of 25-35 adults with various mental illness• Assess and manage mental stability and daily functioning of clientele• Increase functionality through medication, budget, benefit, and general life management• Assist clients in developing treatment plans with concise and measurable goals• Develop strong interpersonal skills in a team working environment

• Manage caseload of 15-20 clients• Work with diverse population of high risk for placement adolescents and their families • Develop treatment goals and evaluate client’s progress throughout the treatment program• Conduct daily home visits with clients and their families• Offer 24/7 on call crisis intervention for all clients• Develop and maintain client files through accurate logging of interactions and by meeting criteria of state laws • Attend DCYF, school, and probation meetings to offer effective advocacy for the best interest of the client and their safety
