USSCRIPT-PBM Title: Clinical Pharmacist Prior authorization review Primary roles: Make recommendations to physicians and other providers for alternative medication therapies based on health plan formulary and current gold standards of practice. Interprets clinical information submitted by requesting physician (or physician representative) along with information in the patient's medical records to make determinations or recommend determinations of medical necessity based on medication utilization and clinical appropriateness in line with current clinical guidelines and evidence based gold standard of care. Provide explanation and interpretations of health plan guidelines and provide recommendations to providers within area of expertise to assist with the peer to peer process. Interacts with providers to gain additional clinical information when needed.

Title:  Clinical Pharmacist-Fulltime.Primary roles: Rotating decentralized clinical staff pharmacist in various units within the hospital. Staffing in central pharmacy which included NICU, general Nursery and pediatrics order entry  and dispensing. Medication management through established pharmacy programs in anticoagulation monitoring program, pharmacokinetic monitoring program and renal dosing program, pharmacist consult service, medication teaching. Provide drug information  to  patients, medical and nursing staff, rounding in critical care units and adverse dug event monitoring. Order entry, medication dispensing, medication reconciliation, participation in hospital medical staff education.

Title:  Pharmacy Clinical Manager.Primary roles: P&T committee presentations , development and management of pharmacy dept performance  improvement programs, drug utilization review, pharmacy and hospital wide protocol development, review  and management of clinical  interventions , policy and procedure review and implementations, development and coordination of pharmacy staff education, participation in hospital medical staff education, monitoring of compliance with established JCAHO quality measures. Implementing, coordinating pharmacy department anticoagulation monitoring program, established and coordinate hospital TPN team and TPN consult service, management of antibiotic pharmacokinetic consult services, participation in hospital interdisciplinary rounds, rounding in critical care units, precept PharmD. student, adverse dug event monitoring, patient discharge medication teaching, participation in various hospital interdisciplinary teams.Order entry, medication dispensing, medication reconciliation.
