Perform yearly health and wellness visits in member’s homes. Some visits were performed via telehealth as well. Review previously identified physical and mental health conditions, current treatments, and medications with each member. Address concerns and help collaborate with the member’s primary care provider to optimize their care. Perform screenings in the home for conditions such as diabetes and peripheral arterial disease in an attempt to catch disease processes at an early stage to avoid preventable progression of the disease. Focus on preventative health screenings and things each member can do to remain healthy and in their own home. Make appropriate referrals to maximize the patient’s health and wellbeing.
